To begin psychiatry care at The Sunroom, please complete the intake form below. This information allows our clinical team to better understand your needs and determine the most appropriate services for you. Once submitted, a member of our team will reach out to you to guide you through the next steps of the intake and scheduling process.

The Sunroom Rates
We are currently in network and accepting Blue Cross and Blue Shield PPO for our psychiatric mental health services.
We also work with out-of-network insurance providers and upon request we can provide you with a superbill that can be submitted via your patient portal. Payment reimbursement from your insurance company will go directly to you. We recommend asking your insurance provider if they cover out of network costs. Patients are responsible for paying our fees up front if participating out-of-network or private pay.
This form is provided as a way to make the process of getting accepted into the practice more simple and efficient, but it can only be used if you agree that providing this information does not mean any doctor/therapist-patient/client relationship is being formed, and that no guarantee is provided that you will indeed be accepted into the practice.
If you are or believe you are experiencing a medical or psychiatric emergency, including suicidal or homicidal thinking, side effects to medication, or any other urgent or time-sensitive matter, do not use this service. Instead, call 911 or go to your closest emergency room.
